Blue health information cards will be coming home with your student the first day of school. Please complete this card, and return it to school as quickly as possible. This information enables me to reach you in the event of an illness, or emergency.

If your child will be taking medication during the school day while at Brenham Middle School, please be aware that the medication must be brought to the nurse's office in an appropriately labeled container, with the student's name, doctor's name, and correct dosing instructions. Parents must also fill out a medication permission slip to be kept on file in the nurse's office.
