The Brenham MCJROTC will be going as paper-free as possible this year! To do this, we ask that all new cadets with a printer at home print out the following pages* for your SRB. 



The files are in PDF format, so your computer will need Adobe Reader installed.



There are two seperate files that contain all of the files needed. The 1^st file is for First-Year cadets ONLY. The 2^nd file is for Second-Year cadets ONLY.



*If you do not have a working printer at home, prints will be provided by request.

This print-out is a general permission form for your cadet's participation in MCJROTC. This page requires a parent/guardian signature.

This print-out lists Rules and Policies that are to be followed while participating the the MCJROTC program. This page requires student and parent/guardian signatures.

This print-out is the parental consent form for Marksmanship training. This page requires a parent/guardian signature.

This print-out is for lisiting Emergency Medical Information for your cadet, and for consent to travel to/from athletic events. This page requires a parent/guardian signature.

This print-out is for listing history of any medical conditions your cadet may have. If either starred checkboxes at the bottom of this page need to be checked, you must print out the 'Physical Examination' page and take it to your Physician to be completed.

This print-out is to be taken to your physician and completed.*

*PLEASE NOTE: This page specifically does not have to be printed up or filled out by a physician UNLESS you check the starred checkboxes on the 'Health Appraisal Form'. If this is the case, this page must be completed by your physician, and returned to your SRB.

This print-out will be filled out as the cadet moves forward in the program. This page does not require any signatures.

This print-out will be filled out in class during the Fall/Spring Physical Fitness test. This page does not require any signatures.

**This page is to be printed out by all year cadets**

This print-out explains the Random Substance Abuse Testing Program, clarifying on things such as the procedure and the confidentiality of the program. This page must be signed by the student and the parent/guardian.
